Whether your student is a serious athlete looking for challenging competition and aspiring to continue on to athletics in college, or a recreational athlete looking to grow his or her skills, fitness, and work ethic in a fun, Christ-centered team environment, Heritage Christian Athletics has numerous opportunities for them.

Forms & Documents

All students must have a Charger Card before they will be allowed to participate in practice or games at HCA. To receive a Charger Card all of the following must be submitted to the Activities Director:

  1. Submit information online for Emergency Care Card (All Grades)
  2. Pre-Participation Sports Physical (PPE)  (All Grades)
  3. Parent/Student Concussion Information Release (All Grades) (Print just the last two pages, sign and submit.)
  4. Complete Online concussion course (High School ONLY)
  5. Complete KSHSAA Eligibility Checklist (High School ONLY)
  6. Submit athletic activity fee (All Grades)

The KSHSAA Pre-Participation Physical form is a prerequisite for participating in Heritage Christian Athletics. Please note:

  • Physicals must occur after May 1st of the preceding school year.
  • All pages must be filled out if you are in grades 9-12 but only the first two pages must be completed if you are entering 5th through 8th grade.

We take the health and safety of our student-athletes very seriously. KSHSAA has provided the following resource which all high school athletes are required to complete prior to participating in athletics at Heritage.

Each athlete is required to pay an athletics participation fee according to the schedule below. Payment should be turned in to the Activities Director in the upper school office. There is a maximum of two fees per school year per student.

  • Middle School (6–8): $75/sport ($150 annual max)
  • High School (9–12): $125/sport ($250 annual max)

Sports Medicine

Our high school student-athletes receive professional athletic training support from sports practitioners from our healthcare partner HCA Midwest Health.
